Seam repair
The joint between sheets
On a large low-slope roof the relationship between where water appears inside and where it entered is often weak. Water travels between membrane and insulation, along the deck, and down structure before it surfaces — sometimes a long way from the breach. Investigation means working systematically rather than guessing: flood testing a bay at a time, electronic leak detection, or an infrared survey to map where the insulation is wet.

Almost always because each repair went where the water appeared rather than where it entered. On a low-slope roof that gap can be tens of feet. Until someone maps the wet insulation or tests systematically, repairs are guesses.
Wet insulation holds heat longer than dry insulation, so after sunset it shows up as warm areas on a thermal camera. It maps the extent of saturation across the whole roof in one pass, which is what tells you whether you are facing a repair, a partial recover, or a replacement.

Low-slope roofs in this climate fail at details rather than in the field, and the damage is usually well advanced before water reaches a tenant space.
